Avandia Restrictions Issued
The U.S. Food and Drug Administration has announced that rosiglitazone-containing medications—including Avandia, Avandamet and Avandaryl—will face new restrictions. These medications are used to treat type II diabetes.
The new Avandia restrictions are part of a Risk Evaluation and Mitigation Strategy (REMS), the FDA said in a statement. The REMS program is in place for the management of marketed drugs with serious health risks for the patient. The restrictions being imposed on rosiglitazone-containing medications are due to the fact that data suggests that there is an elevated risk of heart attack in patients treated with the drug.
Avandia Restrictions will require Patients to Enroll In Program
Previously, the REMS for rosiglitazone, which was initially enacted on Sept. 23, 2010, mandated only that a Medication Guide be provided with the medication. Now, the Avandia restrictions will require healthcare providers and patients to enroll in a special program to prescribe and receive rosiglitazone-containing drugs, Avandaryl and Avandamet.
Rosiglitazone is sold as a single-ingredient product under the brand name Avandia; it is also sold as a combination product under the brand name Avandamet (contains rosiglitazone and metformin) and under the brand name Avandaryl (contains rosiglitazone and glimepiride).
According to the REMS, called the “Avandia-Rosiglitazone Medicines Access Program,” limits for the use of rosiglitazone-containing drugs include the following:
- Patients already being successfully treated with these medicines.
- Patients whose blood sugar cannot be controlled with other anti-diabetic medicines and who, after consulting with their healthcare provider, do not wish to use pioglitazone-containing medicines (Actos, Actoplus Met, Actoplus Met XR, or Duetact).
Avandia Restrictions to take affect November 18, 2011
After Nov. 18, 2011, rosiglitazone-containing medications will not be available through retail pharmacies. Rather, patients enrolled in the Avandia-Rosiglitazone Medicines Access Program will receive their prescriptions via mail order, which will be sent to specifically certified pharmacies participating in the program, the FDA explained.
Patients currently taking a rosiglitazone-containing medication should contact their healthcare provider as soon as possible, to determine if they should continue taking their current medication and, if so, to become enrolled in the Avandia-Rosiglitazone Medicines Access Program. The patient’s healthcare provider will have to complete and sign the patient enrollment form.
The FDA advises healthcare providers that they should determine whether their patients should be taking rosiglitazone medications, based on risks and benefits of the drug versus other available medications. Additionally, healthcare providers must enroll in the Avandia-Rosiglitazone Medicines Access Program in order to prescribe the medications.

In addition to the accumulating evidence of its risks, it is not even as effective as other diabetes drugs in lowering blood sugar or hemoglobin A1C, a measure of glucose control.

GlaxoSmithKline Pay Avandia Claims to Avoid Lawsuits
Glaxo officials have agreed to pay at least $700 million over the last eight months to resolve about 12,000 suits alleging the drugmaker failed to warn consumers that Avandia could cause heart attacks and strokes, people familiar with the accords told Bloomberg News earlier.
GlaxoSmithKline to pay 1,000 Avandia Lawsuits
Glaxo, the U.K.’s biggest drugmaker, is in settlement talks with lawyers for an estimated 1,000 former Avandia users who sued the company in state courts in Pennsylvania, California and Illinois, Dianne Nast, a plaintiffs’ attorney who serves on a group helping to oversee cases consolidated in federal court in Philadelphia, said in an interview yesterday.
Avandia Settlements in the L.A. court have paid 2,400
Settlement talks in state court in Los Angeles already have resulted in 2,400 former Avandia users settling their claims, Nast told the judge. The lawyer said she expected the talks to generate about another 200 settlements in state court in Philadelphia and an undetermined number in Illinois.
Glaxo puts 6 Billion in Reserve for Avandia and other Drugs
Glaxo said in January it was taking a $3.5 billion charge to cover expenses linked to investigations and suits over Avandia and sales practices on other drugs. Glaxo’s Diabetes Drug Avandia Pulled From U.S. Pharmacies
Avandia will be pulled from U.S. pharmacies in November and available only through a special program, the FDA said.
The restrictions are part of a risk mitigation strategy established in September with a medication guide to limit the use of Avandia after data suggested patients had a higher chance of heart attacks. The drug and combination therapies containing it will no longer be available in retail pharmacies starting Nov. 18, according to the FDA, which is requiring doctors and patients to enroll in a program to prescribe or take them.
Avandia, once the world’s best-selling diabetes bill at $3 billion in annual revenue, generated $680 million in sales last year for London-based Glaxo. About 460,500 patients filled a prescription for Avandia, Avandamet or Avandaryl at a retail pharmacy from January to October 2010, the FDA said.

GlaxoSmithKline Plc, the maker of Avandia and the U.K.’s biggest drugmaker, said it will have fourth-quarter legal costs of 3.5 billion dollars because of a U.S. investigation into sales practices for certain products and product-liability cases related to the Avandia diabetes drug.
The company is setting aside additional provisions for the investigation by the U.S. attorney in Colorado into the company’s sales and promotional practices of certain products, London-based Glaxo said in an e-mailed statement today. The money also will cover for the “substantial” number of new claims filed in the U.S. regarding Avandia, Glaxo said.
Avandia Withdrawn
The company has stopped promoting Avandia worldwide after regulators said the treatment would be withdrawn from the market in Europe and sales would be limited in the U.S. because of an increased risk of heart attacks. Avandia was once the world’s best-selling diabetes drug.
The U.K. drugmaker has “now completed its assessment of these additional” Avandia cases and made “an estimate of likely future claims,” the company said. Glaxo didn’t say how many Avandia cases remain to be settled and that is a concern.
Avandia Colorado Investigation
The U.S. attorney in Colorado issued a subpoena in 2004 over Glaxo’s “sales and promotional practices relating to nine of its largest selling products,” the company said in its 2009 annual report. Federal prosecutors in Colorado are investigating whether Glaxo promoted the drugs for unapproved uses, the company said.
The investigation, which is nationwide in scope, also looks into ways that Glaxo potentially influenced doctors, including company-sponsored medical education programs, speaker events, advisory boards, speaker training programs, clinical studies and “related grants, fees, travel and entertainment.”
Glaxo hasn’t disclosed which products are the subject of the investigation. Alex Harrison, a spokeswoman for Glaxo, declined to comment in an e-mail.
